To apply for this sale, complete the following form, print it, and mail it with your check made out to OVCAG to the address below. Registration fees are based on the number and size of tables requested – see below. (Please do not send cash.)

The Radisson Hotel, Sunnyvale, the Orchard Valley Ceramic Arts Guild, their officers, volunteers, and members assume no responsibility for loss, theft, or breakage of any items displayed in this show. I understand that pieces are displayed entirely at my own risk and I will not be reimbursed for damaged or missing pieces.
